1 Scope
This document specifies the overall requirements for investigating production safety accidents, including on-site investigation, evidence collection, accident analysis, and the requirements for compiling the accident investigation report.
This document applies to the investigation of production safety accidents.
Note: In this document, "production safety accidents" are referred to as "accidents."

3 Terms and Definitions
The following terms and definitions apply to this document.
3.1 Accident Investigation
A series of tasks conducted to ascertain the course of the accident, its causes, personnel casualties, and direct economic losses, to determine the nature of the accident and assign responsibility.
3.2 Accident Scene Investigation
Activities involving on-site inspection, examination, measurement, and recording of accident locations, items, equipment, etc., for the purpose of collecting evidence and clues.
3.3 Inquiry Interview
An investigative method involving questioning of relevant personnel such as parties involved in the accident, management personnel, supervisory personnel, and rescue personnel to understand the circumstances of the accident.
4 Overall Requirements
4.1 Accident investigation shall ascertain the course of the accident, its causes, personnel casualties, and direct economic losses, determine the nature of the accident, assign accident responsibility, summarize accident lessons, and propose corrective and preventive measures.
4.2 Accident investigation procedures are divided into general procedures and simplified procedures according to regulations. The general procedure for accident investigation includes on-site investigation, evidence collection, accident analysis, and compilation of the accident investigation report. When conducting accident investigation using simplified procedures, the simplified procedures shall comply with the requirements of Appendix A.
4.3 After an accident occurs, the accident scene shall be preserved. Objects, traces, and conditions related to the accident shall not be damaged. If it is necessary to move certain objects at the scene to rescue the injured, the scene shall be marked accordingly and protective measures shall be taken.
4.4 When conducting an accident investigation, an accident investigation work plan shall be formulated based on actual needs.
5 On-Scene Investigation
5.1 General Requirements
5.1.1 On-scene investigation shall clearly define the specific scope, content, route, and safety protection measures.
5.1.2 The number of personnel involved in on-scene investigation shall be no less than two.
5.1.3 Prior to on-scene investigation, relevant personnel such as accident parties and reporters shall be consulted to understand the situation at the scene before and after the accident.
5.1.4 Prior to on-scene investigation, hazardous factors at the scene shall be identified and assessed, and corresponding measures shall be taken.
5.1.5 Investigation personnel shall be equipped with and use necessary personal protective equipment.
5.2 Implementation of Investigation
5.2.1 Investigation personnel shall enter the scene according to the investigation route and shall not damage the original state of the scene. For items hazardous to health, safety protection measures that do not damage the original evidence shall be taken.
5.2.2 The content of on-scene investigation shall include situations such as site damage, failure or damage to equipment and facilities, failure or damage to safety facilities, and the locations of casualties. Specific content may refer to Appendix B.
5.2.3 During on-scene investigation, relevant traces and items remaining at the accident scene shall be collected. Before collection, they shall be photographed or video recorded. After collection, appropriate sealing methods shall be used for preservation, labels shall be affixed, and an inventory shall be completed. Labels and inventories shall be signed or sealed by the collector, witness, or party involved.
5.2.4 On-scene investigation shall include the creation of necessary drawings covering accident-related circumstances, which shall be signed and confirmed by the creator and the date of creation shall be noted.
Note: Examples include drawings of the accident scene, locations of casualties, process (technological) flowcharts, etc.
5.3 Preparation of Investigation Records
5.3.1 Upon completion of on-scene investigation, an on-scene investigation record shall be prepared.
5.3.2 The on-scene investigation record shall document the investigation personnel, investigation time, investigation route, and investigation content. It shall truthfully describe the basic situation of the accident location and circumstances related to the accident, and attach corresponding drawings, photographs, or videos as needed. The content and format are specified in Appendix C.
5.3.3 Personnel participating in the on-scene investigation shall sign and confirm the investigation record.
